Accentra Case Management is a program of the Oklahoma Department of Human Services,
is available to senior and disabled adults eligible for Medicaid services and nursing home
level of care.

As a case manager, we provide some services directly and coordinate others, always ensuring that your need are met.

- Home Delivered Meals
- Homemaking Services
- Durable Medical Equipment
(i.e. bathroom equipment, incontinent supplies)
When you enroll in theprogram, you work with an Accentra Case Manager who visits you at your home to learn about your needs. The Case Manger shares your goal of independent living and through the assessment process, identifies risks to your independence.
We understand your concerns about safety and well being, and we work hard to promote your overall health and maximize your abilities. Your Accentra Case Manager is both an advocate and a care coordinator, assisting you in obtaining necessary medical services including skilled nursing care, medical equipment, home-delivered meals and healthcare supplies.
